Attendant Care Waiver

The Attendant Care Program is funded by the Commonwealth of Pennsylvania Department of Public Welfare to help adults with disabilities improve their quality of life. The goal is to increase their independence, enable them to remain in their home, and seek or maintain employment.

Under the Attendant Care Program, UDS offers a wide range of in-home, non-medical personal care to people with physical disabilities, tailored to meet their individual needs. Our personal care assistants are reliable and compassionate. UDS is a Supports Coordination Agency and Provider for attendant care, certified with the Department of Welfare. We can provide in-home personal care as well as arrange for personal emergency response systems. All of our attendants are trained, bonded, insured and receive PA State Police Criminal and Child Abuse background checks.

Eligibility
Mentally alert adults, age 18 to 59 who are medically determined to have a physical disability expected to last at least 12 months and need personal assistance. Individuals must be able to manage and direct their own care as well as supervise their own attendants. They must also be able to manage their own financial and legal affairs.

Enrollment
A consumer enrolls into the Attendant Care Waiver Program through an approved Attendant Care Enrollment Agency that assesses eligibility and creates the initial service plan. UDS is an approved Enrollment Agency. The consumer's local Area Agency on Aging must also complete an assessment in order to determine clinical eligibility. The consumer now has a choice of approved agencies to provide supports coordination and financial management. UDS is an approved agency. Individuals have the option to receive services through either of the following models:

Consumer Directed Model
Individuals capable of directing their own care, hire their own attendants who can be family members, friends or trusted neighbors. Attendants become employees of the individuals. UDS provides payroll services offered with this model. Spouses are not eligible.

Agency Model
Consumers can choose to receive attendant care services from a variety of Home Health Care Agencies, such as UDS, who are approved Waiver providers.

Combination Model
Consumers can use an agency to provide their attendant care services, as well as have a portion of the services provided by family members, friends or trusted neighbors. Spouses are not eligible.
