COMMCARE Waiver

The COMMCARE Waiver, funded by the Commonwealth of Pennsylvania, Department of Public Welfare, provides services to eligible persons with a diagnosis of traumatic brain injury in order to prevent institutionalization and allow them to remain as independent as possible in their own home.

The COMMCARE Waiver provides a continuum of services, which enables the consumer to choose the model of service ranging from agency (such as UDS) management to complete consumer management of their attendant service.

CSPPPD
CSPPPD stands for Community Services Programs for Persons with Physical Disabilities. These programs are funded by the Commonwealth of Pennsylvania Department of Social Programs and is intended to help people with disabilities remain in their home.

UDS can be the supports coordination agency, or provider of service for the designated service coordination agency. CSPPPD includes the OBRA Waiver, the Independence Waiver, and the Commcare Waiver.

Eligibility
Individuals age 21 or older, diagnosed with a long-term traumatic brain injury that severely limits the person's ability to function in at least three of the following activities of daily living: self-care, communication, self-direction, mobility, learning, or ability to live independently. Individuals must be determined to be financially eligible for special rehabilitative facility level of care, and not be ventilator dependent.
